Job description
- Review medical records and navigate multiple documentation systems to ensure availability of all medical records to assist with completion of health plan required forms, quality/HEDIS measures and closure of gaps in care.
- Support providers/staff by identifying/documenting patient care gaps to be addressed during patient visits.
- Review medical records, collect and enter data with high attention to detail.
- Engage with patients telephonically to coordinate appointments for wellness visits.
- Document with accuracy and detail patient encounters.
- Maintain confidentiality and adhere to HIPAA procedures, guidelines and policies.
- Work with highest degree of professionalism with patients, specialists’ offices, health plan representatives, the medical team and others within the organization.
- Proactively identify through communication with supervisor areas where improvement can be made for increased efficiency and positive outcomes.
- Required Qualifications
- High school diploma
- Ability to multi task and work under narrow timeframes.
- Ability to work independently, self-motivated and organized.
- Professional communication and conduct
- 2 plus years of experience in a medical office
- Other assigned tasks by Supervisor as needed
- Bilingual (Spanish)
Preferred Qualifications
- LVN, foreign doctor, Certified Medical Assistant
- Bachelor degree or associate degree
- Knowledge of electronic medical records preferably ECW and Excel.
- Bilingual (Spanish)
